What to Expect: Our Process
Referral Received
Once we receive a referral, our team reviews the information to ensure we understand the individual’s needs and requested services.
Initial Contact (Within 24 Business Hours)
An Intake Coordinator will contact the client or responsible party within 24 business hours to introduce our agency, answer initial questions, and schedule an intake interview.
Intake Interview
During the intake interview, we gather detailed information about medical history, current needs, preferences, goals, and any safety considerations. This helps us create a plan tailored to the individual
Assessment & Care Planning
Based on the intake, a personalized service plan is developed. We match the client with appropriate staff and outline the frequency and type of services to be provided.
Services Start
Once the plan is approved, services begin. Our team ensures a smooth transition and clear communication from day one.
Ongoing Support & Monitoring
We maintain regular communication, monitor satisfaction, and adjust the service plan as needs change.
